Training Certificate submission

Online submission is only for ANNUAL Health and Safety Training Certificates.  Submit Annual Health & Safety training certificate along with DHS cover sheet.

Do not submit initial Health & Safety Assessment forms online.  Health & Safety Assessment forms must be mailed directly to PATCH.  Any Health & Safety Assessment forms received will not be processed if submitted online.

Submittal of certificates must be accompanied by a Health and Safety Cover Sheet for processing

For expedited processing of your approved Safe Sleep training for DHS licensing requirements, mail your certificate to PATCH. Electronic submission of the Safe Sleep certificate will not be processed right away.  Registry applications and Form 2 should be mailed directly to PATCH.

Disclaimer: If any certificate submitted is illegible or altered, PATCH will contact you and request that you mail the original certificate to be able to recognize the training.
